The Risk of Anaesthesia
It is important to understand that there are inherent risks associated with anaesthesia and sedation. We endeavour to ensure that our anaesthetic techniques and monitoring are extensive and are the safest that can be provided for your pet, and we consider life-threatening risks to be exceptionally low.
However, unpredictable events such as allergic reactions can occur and be of genuine consequence to your pet. One recent report estimated the risks to be one in 1,849 in healthy dogs, one in 895 in healthy cats and one in 137 in healthy rabbits. It is also very important to consider and weigh the benefits of anaesthesia when considering a procedure for your pet. Please feel free to discuss this with us if you are unsure about your pet's procedure.
Pre-anaesthetic Blood Tests
Our on-site laboratory lets us screen for hidden problems before your pet's treatment begins. It helps ensure that organs such as their liver and kidneys are functioning properly. These organs are important for a safe anaesthetic and are difficult to assess without blood tests. I understand that the pre-anaesthetic blood test does not guarantee the absence of anaesthetic complications, it may however, greatly reduce the risk of complications as well as identify medical conditions that could require treatment in the future
Declaration
I hereby certify that I am the owner / owner's agent of the patient described in the form and being over 18 years of age, I have the authority to execute this consent. I consent to the performance of the procedure(s) outlined on this form.
While all professional care and attention will be given to your animal, inherent risks do exist with any procedure, including unexpected complications and loss of life. I authorise the performance of any additional procedures as deemed necessary should unforeseen complications arise. In the event that the veterinary surgeon is unable to contact me on the phone numbers provided, I understand the veterinary surgeon will act in the best interests of my animal.
In consideration of the said veterinary surgeon providing the requisite treatment, I hereby agree to pay to him the prescribed fees, and I further agree to indemnify him, his servants or agents, from any loss or liability which they may incur as a result of any inaccuracy whether intended or otherwise in this my declaration. I understand that payment in full is required at the time of discharge and agree to do so.
